Prior to Your First Visit
Please fill out the new patient forms above and bring them
with you to your first visit. If you prefer you may come in
10-15 minutes early and fill out your paperwork at the
clinic. Our staff is always available to assist you. It is
also very helpful to bring your physician's prescription for
Physical Therapy, if you have one. Bring your insurance card
and a picture ID.
Clients
under the age of 18 will need a legal guardian to
sign for
them. Please bring comfortable, loose clothing and tennis
shoes. If you are being seen for a lower extremity issue
wearing shorts is best. Your Therapist will need access to
the injured area for optimal examination.
First Visit
You will meet with your Physical Therapist in one of our
private treatment rooms for a thorough evaluation,
discussion of your concerns/goals, and development of an
individualized treatment plan. Treatment is usually begun
the first day. The first visit usually lasts 1-1˝ hours.
Follow-up Visits
The frequency and duration of your treatments will be based
on the findings of your initial evaluation and in
coordination with your wishes. Clients often attend therapy
1-3 times a week. We offer flexible hours to accommodate
your schedule. Treatments often include instruction with a
self-care program of proper posture, body mechanics, and
exercises to empower you to maintain long-term health.
Treatments may include modalities such as heat/ice,
ultrasound, iontophoresis, electrical stimulation, and
traction for pain relief. All of our Therapists have
advanced training in manual therapy techniques to address
painful or restricted soft tissue and alignment issues.
Individualized
exercise programs are designed with an emphasis on improving
posture, strength, mobility and functional abilities. We
want to help you be able to do the activities you enjoy and
help prevent future injury/dysfunction.
Communication
You will have opportunities at every visit to ask questions
regarding your condition and voice concerns about how your
condition is affecting your life.
Your Therapist will
provide regular updates on your progress to your Doctor or
Nurse Case Manager as appropriate.
Payment for Services
Please bring your insurance
information to your initial visit. Our staff will
contact the company directly so we can give you a complete
explanation of benefits. You may also choose to call your
insurance company and ask about your Physical Therapy
benefits. Insurance companies require us to collect
deductibles and co-pays. If you are concerned about payment,
please let us know. We are willing to work with you to set
up a reasonable payment plan. We also accept auto claims, liens,
and VA patients. Private pay is available for those without
insurance. |
